Centennial Swim/Dive Parent Checklist for Registration

• Pre-participation Evaluation Form (Physical):Get a physical and have this form filled in and signed by a doctor. This is required.
• Centennial DragonFly Site:Complete and sign all sections of the site.Upload the Pre-participation Evaluation Form that has been filled in and signed by a doctor.Also upload insurance verification or verify Fulton County Insurance.This is required.
• Swim & Dive Registration:Please use the PickleJuice App to complete Registration, Spirit wear sizing, and other Fees. Payment plans are available.This is required.
• Activity Fee: A separate payment is required for the activity fee.Payment is accepted via the link above. This is required.
• Centennial Nike warm-up:At Information Night, sizing specs will be given.This payment is made with the other registration fees.The warm-up top and bottom is required for new swimmers and divers, and optional for returning swimmers and divers.
• Cap selection:Online selection of a silicone cap vs a latex cap
• Centennial team swimsuit:Information for purchase and fitting will be available on Information night.Payment is made directly to the vendor by cash, check, or credit card.This is required for new swimmers and divers.
• Team Parka:Fitting available and pay deposit for parka.The deposit is made by a separate check.This is optional.
• Spirit Wear:Purchase additional team gear for athletes and parents.This is optional.
• Volunteer:We need volunteers at meets and social events.Speak to a board member if you are interested.More information to come.
• Team communication:Last but not least!Please take a moment to register (free) on our CHS Swim & Dive Swimtopia website.This is how we communicate to the team.This is also how the athlete RSVPs to the competitions and parents RSVP for volunteer assignments.
