REGISTRATION FORMS AND LINKS
Here is a list of the forms/links that are needed for registration with a brief explanation of how to fill them out.
Pre-participation Evaluation Form (Physical):All swimmers and divers must have a physical and a physical form signed by a doctor on file to participate in practice and compete in meets. The form needs to be uploaded to DragonFly. GHSA Pre-participation Evaluation Form - Physical
Centennial DragonFly Site: This year Centennial will be using the DragonFly website to collect the required health forms. Please go to www.dragonflymax.com to fill in the forms on the Centennial Dragonfly site. The School Code is TWWLZK.
Swim & Dive Registration: We’re using the PickleJuice App to complete registration, input spirit wear sizes, sign waivers, and process registration fees. This is all completed and paid for online by credit card (extra 2% processing charge). Similar to last year, we are offering payment plans. This app works best on Chrome or Firefox browsers.

COACH
Coach James Jacobi (email: [email protected]) will be the Head Coach this season. He teaches in the Social Studies department at Centennial-Room H-56.
PRACTICES
High School Only swimmers: Practices will be at the Roswell Adult Center.
oPractice Begins – Wednesday, October 14th @Roswell Adult Center: 200 Frank Lewis Drive, Roswell GA 30075
oPractice times: 6:15 am -7:15 am; specific varsity/JV days per week yet to be determined. **There are COViD-19 limits requiring no more than 20 swimmers per practice, so final practice dates will be set after registration.**
Divers: Practice location/times to be determined.
Year round swimmers: You will continue to practice at your swim club.
TEAM GEAR
All NEW swimmers/divers will be required to purchase a new swimsuit and a black Under Armour warm up top. Purchase of the swimsuit and warm-up is optional for returning swimmers and divers since you may use the items you purchased in previous years.
Swim Suits:
Swimmers: New swimmers are required to buy and wear the team swimsuit. Please do not practice in the new suit. Wear these suits to meets only. This is because the suits stretch out quickly and wear out quickly if worn for practice AND the meets.
Divers: New divers will also be purchasing the new team suit. We have been told that it is a choice that works well for both diving and swimming.
The vendor (All-American) supplies our suits. Normally, on Information Night, the swimmers have the chance for suit fittings and the vendor takes payment. This year is not a normal year.
We do not have a sizing kit available but all sizes and styles are available at the store and orders can be placed there. If you are confident in size and want to order you can call the store or Rachael King will be gathering orders, but please note there are no returns.
There is a bit of urgency, as the suits need to be embroidered, so please get these fitted ASAP.
Athletes requiring suits who need fittings will need to visit the All-American store, 885 Woodstock Rd, Suite 400. Roswell, GA, 30075. Phone : 770-400-9870
The team suit is the Speedo Solid Lycra Black with the CHS logo: the cost is:
Female Suits: $52.92 for Proback or Flyback
Male Suits (jammer): $35.64
Male Suit (brief): $ 32.40
Team shirt: Because of the unusual season, there will only be team shirts this year, no additional items are for purchase. Team shirt sizes Adult S-XL (included)
Centennial swim cap- must be ordered for each swimmer. There is a choice of silicone or latex. Both are available to check out at All American Swim. (included)
Warm Ups: Jackets and pants sizing for the Under Armour warm-up suit will be announced during the Information Night so that you can order the correct size. The Warm Up top is required for new swimmers and divers and included in the registration payment.
Fins (required for Centennial swimmers): Again this year is a requirement for use of fins at swim practice. When registering, select the appropriate size. There is no fee for the use of the fins. However, if the fins are not returned at the end of the season a $20 fee will be required.
Swim Parkas (Optional): Parkas are long, oversized, hooded coats for swimmers/divers to help keep them warm at meets and practices. A $100 deposit is required for the swim parka for the season. The deposit/check will be returned to you at the banquet or when the parka is returned. Swimmers may only wear the parkas to meets and practices. Any parka not returned or damaged will be assessed $125 replacement fee. Parkas will be distributed at registration night and not information night.. Swimmers are NOT required to check out a parka for the season. Any solid Navy or Black can be used as long as it does not have another club team logo. Some swimmers do not use a parka at all.
VOLUNTEERS AT MEETS
The team is hosting several dual meets this year. We will need volunteers (timers, computer operators, etc.) to help these meets run smoothly. The meets usually last a couple of hours and the schedule will be released shortly. Please consider volunteering. More information to come as the season progresses.
SOCIAL EVENTS
The team has social events during the season. Pasta dinners have been a staple of the high school swimming social calendar. Information on this is TBD.
